If the number of people at your site changes during the period you are taking the survey, or the employee numbers provided were incorrect and you would like to change them, please click here to request a change in the platform.
Please note: If the change is small, or the survey has already been completed, this is not necessary.
If a site champion changes and a new person needs access to the data, or an existing site champion has a new email address, please click here to request a change to the user account.
